Serenity Hospice ATX, LLC (Serenity Hospice Central Texas) is a hospice provider. This Notice of Privacy Practices explains how we may use and share your protected health information (PHI) for hospice treatment, payment, and health-care operations, and for other uses the law allows or requires. It also describes your rights.
Treatment. We use PHI to provide hospice care wherever you call home — including a private residence, assisted living, nursing home, or memory care. We share information with our nurses, aides, social workers, chaplains, physicians, your attending doctor, pharmacies, and other caregivers involved in your hospice plan of care.
Payment. We use PHI to bill Medicare, Medicaid, and other payers for hospice services and to obtain payment.
Health-care operations. We use PHI to run the hospice: quality review, staff training, licensing and surveys, and other business activities needed to provide care.
We may also share PHI when the law requires it — for example public health, health oversight, abuse or neglect reports, judicial and administrative proceedings, law enforcement, coroners and funeral directors, organ donation, research under applicable rules, workers’ compensation, and national security. We must share information with you and with the U.S. Department of Health and Human Services when they review our HIPAA compliance.
Other uses and disclosures not described in this notice generally require your written authorization. You may revoke an authorization in writing, except to the extent we have already relied on it.
We are required by law to maintain the privacy of PHI, to give you this notice, and to notify you if a breach of unsecured PHI occurs. We may change this notice. The new notice will apply to PHI we already have. The current notice will be posted on this website and available from our office.
If you believe your privacy rights have been violated, you may complain to us or to the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint. To complain to us, contact:
