Will and testaments, estate plans, and other relinquishing of control of assets can be very important decisions to make prior to passing. However, we understand that not everyone has the time or knowledge of how go about making these preparations. At Serenity Hospice, we can provide patients with the social services they need to leave all their affairs in order.
To best provide for our patients, ensure their wishes are heard, and their needs are accurately cared for and carried out, Serenity Hospice connects them to one of our experienced and skilled hospice social workers. Hospice social workers play an important role in the lives of our patients. As a patient advocate, they care for and represent both the patient and their family throughout the end of life process. Our social workers aim to understand patient’s needs and desires, as well as those of their family.
Our social workers also communicate with medical staff to ensure that a patient’s physical comfort, emotional comfort, mental health, spiritual state, and the like are balanced and in good standing. Our social services including counseling, referrals, and coordination with other resources to help patients and their families prepare and make the necessary adjustments prior to death. From start to finish, they are making sure the patients in their roster are receiving the right care in all aspects, as well as assisting with bereavement after the loved-one has passed.

- Emotional support
- Help with long-term care paperwork (such as Medicaid)
- Medical power of attorney
- Life ending important documents
- Advanced directives
- Out of hospital DNR (do not resuscitate)
